Membership Change Request Form

Please review the information below before submitting a request.

📅 Cancellations & Freezes (All Memberships)

  • Cancellation requests must be received by the 25th of the month to take effect the following month.

  • Freeze requests must be received by the 25th of the month to take effect the following month.

  • Memberships may be frozen for $12 per member, per month.

🩺 Medical Freezes

  • Freeze fees may be waived for medical reasons.

  • Medical documentation must be emailed to billing@sender.one before the 1st of the month.

  • While a membership is frozen for medical reasons, facility access is not permitted.

  • Memberships must be unfrozen in person at the gym on your first visit back.

⏳ Billing Support

  • Please allow up to 5 business days for a response to billing inquiries.

Youth Program Members

📅 Cancellations

  • Cancellation requests must be submitted one full calendar month in advance. Example: To leave in March, your request must be submitted before February 1.

❄️ Freezes & Re-Enrollment

  • Freezes are not approved unless for medical reasons (see medical policy above).

  • If you cancel and later wish to rejoin, the re-enrollment fee is reduced to $39.
